Publication in the South African Orthopaedic Journal

October 2024 

Dr Sven and Dr Chris, along with their colleagues, recently published their article in the South African Orthopaedic Journal. Not only did they publish, they also won the prize for the best paper published in the journal in the preceding year. Well done!

Surge's First Publication

September 2024 

We are excited to announce that we have published our first article in the Journal of Arthroplasty. In this article, we chose to focus on the outliers – the 6% of patients who did not go home on the same day as they were intended to. We learnt a lot about our processes and progress thus far when writing the article. We hope that other departments implementing Same-day arthroplasty pathways can moderate their learning curves by learning from our outliers. Martin's Masters Graduation

July 2024

We are very proud of Martin for graduating with his Masters degree from the University of Witwatersrand. His thesis was entitled “Patients’ perspectives, pain and health-related quality of life in same-day discharge hip and knee arthroplasty”. We will certainly look to his research to help us further improve the recovery process for our same-day replacement patients. Congratulations on your hard work and contribution to this specialised field of interest, Martin! 

Value-Based Care Session: South African Arthroplasty Congress

The Team was invited to present a Value-Based Care session at the 2024 South African Arthroplasty Society Congress. We put together a team of local and international experts, all who understand value from different stakeholder perspectives. Our colleagues and friends Dan Hoeffel and Karl Koenig joined us from the U.S. to present their expertise and experiences.  

World Physiotherapy Congress: Dubai

June 2023

Martin has been completing his Master’s degree in Physiotherapy with focus on Same-Calendar-Day Discharge Arthroplasty. He was given the opportunity to present his work on an international stage at the World Physiotherapy Congress in Dubai. His research strengthens our interdisciplinary insight into Same-Calendar-Day Discharge Arthroplasty and we look forward to reading his final dissertation.  We are sure the cap and gown pictures will follow soon!

EFORT Congress 2023: Vienna

May 2023

The Surge Research Team headed off to Vienna for the EFORT Congress to present eight abstracts that were accepted into the conference. Two podium presentations were delivered by Duwayne and Theo and six poster presentations were displayed. To view the posters that were presented click here  This was our first experience on an international stage and we look forward to further opportunities to present our work to international colleagues.

The focus of the evening was Same-Calendar-Day discharge arthroplasty and the integration of primary and specialised care. These events improve coordination of care and communication between the disciplines and create a space for innovative discussions and networking. Physiotherapy Mastered

June 2022

Retha-Mari Prinsloo (Physiotherapist) has recently completed her Masters in Physiotherapy Orthopaedic Surgery. The study assessed the Same-Calendar-Day Discharge Arthroplasty pathway from a physiotherapy and rehabilitation perspective. Retha published two articles during her study period which can be accessed below.

“I started my Masters in Physiotherapy in 2020 after I realised how limited the research was especially in SA regarding the role of Physiotherapist as part of the multi-disciplinary team in an accelerated rehabilitation pathway (ARP) for hip and knee arthroplasty patients. I specifically looked at proposing a detailed Physiotherapy protocol since I could not find any in published literature. Further I did my research on the effect of early mobilisation and increased frequency of Physiotherapy on post-op day zero (POD0) on outcomes following hip and knee arthroplasty. We demonstrated that Same-Calendar-Day Discharge Arthroplasty is possible in SA in patients following hip and knee arthroplasty by implementing a multidisciplinary ARP. Physiotherapy with patient education, early mobilisation and increased frequency of treatment on POD0 and as part of this ARP led to improved patient-reported outcomes and satisfaction in a safe and cost-effective manner. The plans for the future are to continue to investigate and answer research questions and possibly attain my Ph.D in this field of research.”

Visit to the University of Texas, Austin

Four of our surgeons completed a Master’s in Healthcare Transformation through the University of Texas, Austin. They graduated in 2021 and were not able to meet their fellow students and lecturers in person due to the Covid travel restrictions. In May 2022 they were invited to attend and present at a reunion function. Dr Duwayne Vermaak presented his work on the integration of a primary care provider into a specialist practice and Dr Toni Monni presented on patient-centered care.

Our surgeons presented six abstracts at the SAAS Congress in March. It was a privilege to be able to share our research with our colleagues and be able to learn from international and local arthroplasty specialists.  We will continue to embrace these learning opportunities to improve the way we serve our patients. Recent media coverage has reignited the discussion around the outcomes of Total Knee Arthroplasty (TKA). Both total knee and hip arthroplasty have transformed the management of patients with advanced osteoarthritis, offering pain relief, restored function, and improved mobility. However, global data shows that, in most cases, the short-term outcomes of Total Hip Arthroplasty (THA) outperform those of Total Knee Arthroplasty (TKA), with success rates of 96% for THA compared to 89% for TKA. Several factors contribute to this disparity.

The knee joint’s mechanics are influenced by three essential factors: morphology, soft tissue balance, and alignment. These elements are intricately linked, each affecting and depending on the other to produce personalized joint movement, or kinematics, for every patient. When even one of these factors is altered from its natural state, the balance is disrupted, leading to potentially suboptimal outcomes. This is why personalized goal setting is critical—every knee has its own unique characteristics, and what works for one patient may not work for another.

There are two main approaches to knee replacement: kinematic alignment and mechanical alignment. Although they begin with different strategies, both aim to achieve the same outcome: a well-functioning, pain-free joint. Unfortunately, current implants and techniques have yet to perfectly deliver this outcome for every patient.
